Physical Evaluation Required for Students to Participate in Sports.

An annual athletics physical form must be submitted prior to participation in athletics. The athletics physical forms are only valid for one calendar year from the date of exam.

The health record form is not an acceptable substitute for the athletics physical. The athletics physical has a 3 page personal health questionnaire and confirmation from your child's physician that he/she is medically cleared to participate in sports without restriction. (Health record and an athletics physical are not interchangeable.)
