New Student Mandatory Health Documents

If your student is new to Damien, below is a list of all health documents required by this health office prior to their first day on campus:

1. Student Health Record (form 14)

2. TB Risk Assessment Form

3.  Immunization Record (if not recorded on the Student Health Record, doctor's office to provide)

4. Athletic Physical (this is only required if your student will be participating on a Damien athletic team)

5. Emergency Rescue Medicine Form (only required if your student may require a life saving medication while at school like epipen, glucagon, asthma inhaler, etc. or if they need assistance with required daily medicines such as for attention deficit disorder)

Once the forms are filled out, please email to lyewchuk@damien.edu or health@damien.edu, or drop off in person at our Welcome Centre during business hours.

Please note that all 7th grade students must have an up-to-date physical exam (regular or athletic physical) and by state law, ALL new incoming students (all grades) must have TB clearance on file the first day of school.

If your student is returning to Damien, please ensure your health documents are up to date and complete.
